Our Community Wellbeing Program offers support to local residents 65 years or over who are registered with My Aged Care, or under the age of 65 receiving a disability support pension and eligible for SA HACC services.
Our services are subsidised through the Commonwealth Home Support Program and the Home and Community Care Program and are designed for residents to maintain their independence, stay in their own homes longer and increase wellbeing.
If you or a family member or friend require information or assistance with accessing services then please contact us. Our knowledgeable and friendly Community Wellbeing team can provide you with information and connect you to services that support you to live well.
My Aged Care is the single entry point for accessing aged care services in Australia. To register with My Aged Care phone 1800 200 422 or visit the website https://www.myagedcare.gov.au/
Making Every Contact Count (MECC)
The City of Holdfast Bay has been chosen as the first metropolitan council in Australia to participate in the MECC (Making Every Contact Count) initiative. In partnership with SA Health and Meals on Wheels we are having conversations with older adults about lifestyle changes that can make the greatest improvements to health and independent living. Find out more about MECC here
Complaints – Commonwealth Home Support Program
If you’re concerned about the care or services you receive from the Commonwealth Home Support Program, you can make a complaint. This way, providers can improve the quality of care and services they provide to you, your loved one or anyone else.
To register a complaint or for more information, go to the Aged Care Complaints commissioner.
